Revive Dental Studio is a private dental practice. We are the data controller for the personal information we hold about you, which means we decide how and why it is used.

Through this website we collect only what you give us when you send one of our forms: your name, your contact details, and what you tell us about what you need. The appointment request form may also ask for your date of birth; the referral forms ask a referring dentist for their professional details and the clinical reason for the referral; the self-referral and referral forms allow photographs, letters or x-rays to be attached.

To provide safe dental care we also keep an accurate clinical record once you become a patient. That record is held in our practice systems, not on this website.

We use what you send through this website to respond to your enquiry, to arrange an appointment or consultation, and to coordinate a referral with the dentist who made it. Our lawful bases are the provision of healthcare, our legitimate interests in running the practice and, where we ask for it, your consent.

We will only contact you about your enquiry, appointments and matters relating to your care unless you have told us you would like to hear from us about other services.

We never sell your information. We share it only where it is necessary for your care or required by law: for example with a dental laboratory, with a specialist you are referred to, or with a dentist who has referred you to us. Where a referral is made, we share the clinical information needed for that treatment and no more.

The website is hosted by a trusted supplier that processes data on our behalf under a written contract that meets UK GDPR standards. Form submissions are stored on the website so that nothing is lost in transit, and are accessible only to staff who sign in.

Website enquiries are kept for as long as needed to deal with them and are then removed from the website. Information that becomes part of your clinical record is kept in line with professional guidance for dental records.

You have the right to ask for a copy of the information we hold about you, to have it corrected, and in some circumstances to have it erased or its use restricted.
